An in-person visit at an Ascension Via Christi location is required to sign up. You can sign up now at any of our ministry locations.

  • To sign up for a portal for your doctor office visits, call your Ascension Medical Group doctor’s office or simply talk to them when you are there for a visit.

1. Can anyone sign up for access to their patient portal?

Ans: You must be 13 years of age or older to sign up for a patient portal.

2. Can I enroll my child for their patient portal?

Ans. You may sign up for access to your child’s information if they are under 13. Once they reach age 13, a child can give “proxy” access to another adult. Once a person turns 18, they need to register for their own account by talking to their healthcare provider.

3. Will my information be kept confidential?

Ans. Like your paper-based record, your EHR is protected by the Health Insurance Portability and Accountability Act (HIPAA). Your privacy is very important to us. Rest assured your health information is always kept confidential.

4. Can I share my password with a family member so that he/she doesn’t need to sign up for proxy access?

Ans. This isn’t recommended. Each person should have his or her own account. That way, you will always have the choice in the future to stop sharing your information with another person.
