How to Signup on Summit Health Patient Portal
To sign up for a Summit Health account, click the “Log in to Summit Health patient portal” button above and select “Create An Account” or contact your provider’s office and ask them to send you an email invitation. The email will contain a secure link that will allow you to complete the registration process.

- Provide your password and click on “Continue”.
- If you have an Activation Code, you can use it to sign up.
- If you don’t have an Activation Code, contact your practice or provider to obtain an Activation Code.
Please note: Registration is only available for laptop or desktop computer users. It is not available on mobile devices at this time. You may also contact your Summit Health provider’s office to request an email invitation to the patient portal.
